Medicare - Participating Provider. Freedomed accepts Medicare assignment.
Private Insurance/Major Medical/Group Health Plans: Freedomed does not accept HMO's.
Upon receipt and verification of insurance information, Freedomed will contact you to let you know if your insurance plan is accepted and if there will be any cost to you. Please note: since Freedomed is a provider with most insurance plans, the majority of patients have no out of pocket expenses at all! Upon receipt of the completed Medical Authorization form and the Physician's Order from your doctor Freedomed will ship your first 90-day supply to your home. Following, Freedomed will contact you once every 90 days to discuss your supply needs before shipping your next order.
Supplies Provided:
The types of supplies that will be sent to you are based on the supplies requested, your physician's consent and your insurance coverage. If there are supplies requested that are not covered by your insurance, you will be contacted by Freedomed prior to any supplies being shipped. For Medicare patients, please note that Medicare does not cover the cost of prescription items, but they do cover testing supplies.
