General Conditions
These Terms and Conditions of Use (the "Terms") apply to your access and use of the MyHealthSupplies world wide web site, located at www.myhealthsupplies.com ("MyHealthSupplies"). All information and Content (as defined in these Terms) on the MyHealthSupplies web site (together with the design, layout and organization and the underlying source HTML files that implement the hypertext features, hereinafter referred to as the "Site") may be used solely under the following Terms. By accessing, browsing or using the information or services on the Site, you (the "User") expressly agree to and consent to be bound by the Terms in full. Purpose and Use of the SiteThe general purpose of the Site is to provide Users with information about health-related products and services. The specific information provided by MyHealthSupplies is set forth in detail in the Site (collectively referred to hereinafter as the "Services"). User's right to access the Site and the Services is subject to any limits established by MyHealthSupplies. MyHealthSupplies is purely an information facilitator and does not process product/service orders or inquiries. The Site exists solely to provide information about providers of health-related products and services. As a going concern, The Site is affiliated with the third party providers only to the extent that it receives commissions on qualifying transactions. User further understands and agrees that from time to time the Site may be inaccessible, unavailable or inoperable for any reason, including, without limitation: (i) equipment malfunctions; (ii) periodic maintenance procedures or repairs which MyHealthSupplies may undertake from time to time; or (iii) causes beyond the control of MyHealthSupplies or which are not reasonably foreseeable by MyHealthSupplies, including, without limitation, interruption or failure of telecommunication or digital transmission links, hostile network attacks, network congestion or other failures. The Site and the Services currently are intended for use by Users located in the United States ("U.S."). MyHealthSupplies makes no representation or warranty that information on the Site is currently appropriate for any other jurisdiction.
Third Party Content and TrackingMyHealthSupplies primarily operates under a performance marketing business model and is therefore compensated within a commission-based structure. While MyHealthSupplies does not use cookies, third party tracking technology contained within hyperlinks uses cookies that track commissionable actions by assigning an anonymous identifier to the end user's web browser. Commission Junction and other performance-based marketing networks and services do not use tracking technology as an analysis tool for profiling online customer use. Most browsers are initially set up to accept cookies. Users can change their browser settings to refuse all cookies or indicate when a cookie is being sent.

All actions, claims or disputes arising hereunder or relating to the Site shall be exclusively subject to the jurisdiction of the federal and state courts in the City of Minneapolis, State of Minnesota. User further agrees and expressly consents to personal jurisdiction over User in the federal and state courts in the City of Minneapolis, State of Minnesota, in connection with any such dispute including any claim involving MyHealthSupplies. You and MyHealthSupplies agree that any cause of action arising out of or related to this Service must commence within one (1) year after the cause of action arose; otherwise, such cause of action is permanently barred. If any provision(s) of the Terms and Conditions is held by a court of competent jurisdiction to be contrary to law, then such provision(s) shall be construed, as nearly as possible, to reflect the intentions of the parties with the other provisions remaining in full force and effect.
